2、 in order for blade sets that have been in use for extended periods of time.Recommendation(s): As a result of a blade failure incident, an aggressive blade inspection program, to include NDE, was implemented. During normal tunnel shutdown, a certain percentage of blades were pulled and thoroughly ex

3、amined. Over a set period of time, this program insured inspection of all blades. New blades were equipped with frangible tips.
