2、rce their company safety program, as defined in their Safety drawings are not promptly updated, plus one-line (red-line) drawings do not always contain all electrical circuits.4. The existing facility drawings were not provided to the contractor.Find B: Project submittals from the contractor to the

3、government are a requirement. NASA needs to ensure the required submittals have been delivered, reviewed, and accepted before allowing the work to proceed. The following evidence supports this finding: 1. The contractor failed to develop and submit a point-to-point schematic to NASA prior to “demoli

4、tion” of existing wiring, as required by the construction drawings.2. NASA did not confirm delivery of the point-to-point schematic required by the construction drawings.3. The contractor had no formal or organized process to plan or review its days work.Lesson(s) Learned: All drawings available for

5、 the project should be provided to the work team and, if the drawings have not been verified as complete and accurate, NASA should make the work team aware of this.Recommendation(s): 1. NASA should require A/E firms to develop detailed circuit schematics as part of construction drawings.2. 3. NASA s

6、hould ensure that facility drawings are accurate and are provided to construction contractors when appropriate.4. Construction of Facilities (CoF) project budgets should include fenced funding for redlining and updating existing facility drawings associated with the construction.5. NASA should ensur

7、e that contract deliverables are provided by the contractor at the appropriate time and not allow work to continue unless these are provided.6. NASA should require that safety and job hazard analyses be performed and not allow work to start until these are submitted and reviewed.7. NASA should requi

8、re Pre-Project Safety Reviews prior to construction start. This should be more comprehensive than pre-construction meetings with a format analogous to an Operational Readiness Inspection (ORI), Test Readiness Review (TRR), etc.8. NASA should require that construction contractors conduct daily planni
