2、ract screws. While attempting to remove the screws the Allen wrench broke in half resulting in the individuals hand to come down on the broken sharp end of the Allen wrench shaft puncturing his hand.Lesson(s) Learned: Sufficient downward pressure on the wrench and high torgue could cause flexing or

3、bending of the shaft, which in combination with the applied torque could cause the forged shaft to break suddenly and without warning.Recommendation(s): 1. Take a minute or two, to think about the job you are preparing to do; analyze the risk and danger.2. Wear proper Personal Protective Equipment (

4、PPE), i.e. eye protection, gloves, etc.3. Select the right tool for the job. Use the tool as it was designed to be used. Do not modify the tool or use “cheaters“ to provide leverage or forces the tool was not designed for.4. Remember, everything can and probably will break at sometime, especially if
