A number in parentheses i

Asuperscript epsilon () indicates an editorial change since the last revision or reapproval.INTRODUCTIONMany organizations have been working in the Search and Rescue (SAR) community using theIncident Command System (ICS) framework. In doing so, these organizatio
3、ns have adapted theexisting ICS forms to fit their needs. They have also found that some new forms needed to bedeveloped in order to address problems or areas not considered in the fire-oriented forms. Some ofthese organizations have developed their own standard packet of forms that will address the
4、 typicalneeds of that organization. By addressing the appropriate actions called for by the average mission,these forms can focus the team members activities into a standard operating procedure. This guidewill show examples of form packets used by some SAR organizations.1. Scope1.1 This guide gives
5、examples of forms used in the SARcommunity.1.2 It is not the intent of this guide to recommend one formover another, but to make the user aware of the many differenttypes of forms used. This guide does not purport to containevery form used in SAR, only a few examples of forms in eachcategory.1.3 The
6、se forms cover a great variety of the many aspectsinvolved in SAR. This guide will attempt to give a few versionsof forms used for each aspect identified. The user may choosewhich form best fits his particular need.1.4 This standard does not purport to address all of thesafety concerns, if any, asso
7、ciated with its use. It is theresponsibility of the user of this standard to establish appro-priate safety and health practices and determine the applica-bility of regulatory limitations prior to use.2. Referenced Documents2.1 The forms in this guide have been submitted by mem-bers of various organi
8、zations who are presently using them. Insome cases the organization logo will identify the source of thesubmitted form. Use of logos on forms in this guide does notconstitute an endorsement by either ASTM or the contributingSAR organization. Use of these displays are for the conve-nience and informa
9、tion of the user.2.2 ICS National Training CurriculumICS Forms Cata-log23. Significance and Use3.1 This guide will give SAR personnel options in choosinga form that will fit their specific need. These forms will assistin the organization, management, and documentation of asearch or rescue incident.3
10、.2 Additional forms will be categorized by topics such asmanagement, investigation, training documentation, equipmentmaintenance, and reports. This guide will compare the originalICS forms with samples of those developed to parallel them forSAR.3.3 Once categorized, an explanation will be given for
11、eachtype of form. Some examples of these forms will be shown.Some contributors have included detailed instruction for theuse of their forms.3.4 This guide may serve as the basis for new forms to becreated using some information found here.4. Summary of Guide4.1 This guide has been arranged so that t

Existing ICS Forms5.1 In adopting the incident command system as the pre-ferred method for managing a search or rescue incident, wehave also adopted the forms that go with that system. All the
17、forms included in the ICS are shown for the reference of theuser. In many cases the forms show aparallel to ICS by using the number or the name that corre-sponds to the ICS system.6.2 Forms included in SAR/ICS sections:6.2.1 201 Incident Briefing FormsThis is a form to gatherbasic information, including but not limited to the situation, thesu
21、bject, the overhead team, and initial response actions.
