The procedures chosen are limited and must be seen as very special aspects, generally embedded in the wider scope of classification requirements. Applicable and superimposed

17、yor, the following list, grouped under relevant conventions, indicates those deficiencies which may be considered of such a serious nature as to be categorized as major deficiencies. Absence, substantial deterioration, or defective closing devices, hatch closing arrangements, and watertightlweathertight doors. 9 HULL AND STRUCTURAL SURVEYS 01-02 Basic Definition 1 For the application of SOLAS: the expression ships constructed means ships the keel of which are laid

26、or which are at a similar stage of construction. the term a similar stage of construction means the stage at which: 1. (a) construction identifiable with a specific ship begins; and 2. (b) assembly of that ship has commenced comprising at least 50 tonnes or one per cent of the estimated mass of all

27、structural materials, which is less. In connection with the application of amendments to SOLAS “Date on which keel was laid”, etc. is to be determined according to the followings. 1. 2. 3. I 4. “Date on which keel was laid”, etc. should be confirmed in a building site according to the following -3.

This confirmation should be recorded by, for example, signedktamped shipyards application slips which are duly storedlpreserved together with relevant survey records. “Date on which keel was laid” means only the date on which keel of the ship in question is actually laid down on a slip-way. I

29、t is not applicable in general to the ships constructed by “block assembly”. “A similar stage of construction” means the stage when construction identifiable with a specific ship begins, and assembly of that ships structure comprising of at least in total 50 tons or one per cent of the estimated mas

30、s of all structural material, whichever is less, has been almost completed in a slip-way, dock, assembly shop, etc. The column for “keel laid” in Particular of Ship, Survey Record, etc. should be filled with the above date in cases where statutory certificates (SC, SE, SR, IOPP/suppl., IBC, BCH, IGC

31、, etc.) with “date on which keel was laid” and “a similar stage of construction” are issued, or the said date determines whether the amended rules are to be applied or not. 2 Date of Build (IACS Procedural Requirements No.11) The Class Certificate and the Societys Register of Ships shall indicate th

32、e Date of Build as defined below: For New Construction The year and month at which the new construction survey process is completed shall be specified as the “Date of Build”. Where there is substantial delay between completion of construction survey process and the ship commencing active service, th

33、e date of commissioning may be also specified. 10 PART BO1: GENERAL After Modifications After modifications are completed, the “Date of Build” shall remain assigned to the ship. Where a complete replacement or addition of a major portion of the ship (eg forward section, after section, main cargo sec

34、tion) is involved, the following shall apply: the “Date of Build associated with each major portion of the ship shall be indicated; survey requirements shall be based on the “Date of Build” associated with each major portion of the ship; survey due dates may be aligned at the discretion of each Soci

35、ety. 0 0 0 01 -03 Identification of a Substandard Vessel IMO Resolution A.787(19) as amended by Res.A.882(21) states that a ship can be regarded as substandard if the hull, machinery, equipment, or operational safety, is substantially below the standards required by the relevant conventions or whose

36、 crew is not in conformance with the safe manning document, owing to, inter alia: 1. the absence of principal equipment or arrangement required by the conventions; 2. non-compliance of equipment or arrangement with relevant specifications of the conventions; 3. substantial deterioration of the ship

37、or its equipment because of, for example, poor maintenance; 4. insufficiency of operational proficiency, or unfamiliarity of essential operational procedures by the crew; and 5. insufficiency of manning or insufficiency of certification of seafarers. If these evident factors as a whole or individual

38、ly make the ship unseaworthy and would put at risk the ship or the life of persons on board or present an unreasonable threat of harm to the marine environment if it were allowed to proceed to sea, it should be regarded as a substandard ship. 01-04 Preparation for Surveys 1. Safe conduct of Surveys

39、It is essential for surveyors to pay sufficient attention to the matters on safe conduct of surveys, and also, the person responsible for safety (General manager, Manager) at Survey Offices to which surveyors belong is to pay special attention to the precautionary points as mentioned in -1. to -4. b

40、elow for making sure of the surveyors. 1. 2. 3. Periodical education for surveyors regarding safety Periodical investigation of manufacturers, shipyards, etc. in the territory regarding the safety management related to survey execution Meetings on request made to a person responsible for safety in t

41、he manufacturers, shipyards, etc. , for corrective/preventive actions taken HULL AND STRUCTURAL SURVEYS resulting from observations of poor preparation for surveys with regard to safety management Measures such as a report to the Survey Department, an advice for surveyors of attention and a rejectio

42、n of surveyors attendance, if necessary, in cases where the above requests are not accepted and it is judged that the danger is serious and not eliminated. 4. 2. Surveys on “Cherry-picker” When a “cherry-picker” is used in surveys, its construction and inspection/maintenance standards are to be in a

43、ccordance with the safety standards prescribed by a recognized national organization. When compliance of a “cherry-picker” with such standards is not ascertained, instructions from the Head Office of the Society are to be observed for the construction and inspection/maintenance standards. In practic

44、e, the standards for operation within the cage are to be observed, paying careful attention to ensuring operational safety. 3. NOISE Noise within a confined space can be amplified by the design and acoustic properties of the space. Excessive noise can not only damage hearing, but can also affect commu

46、nication, such as causing a shouted warning to go unheard. 4. SLICKNET SURFACES Slips and falls can occur on a wet surface causing injury or death to workers. Also, a wet surface will increase the likelihood for and effect of electric shock in areas where electrical circuits, equipment, and tools ar

47、e used. 5. FALLING OBJECTS Workers in confined spaces should be mindful of the possibility of falling objects, particularly in space which have topside opening for entry, and where work is being done above the worker. PERSONAL PROTECTIVE EQUIPMENT (PPE) PPE is traditionally regarded as the last line

48、 of protection with the emphasis being place on avoidance and appropriate managerial control methods. However, the potentially hazardous nature and isolated position of those entering a confined space means that, for the surveyor, PPE may be the first line of protection. Each confined space will pre

49、sent different hazards and degrees of risk to health and safety, the final provision of PPE should therefore be based on an assessment of risk. As a general rule the following guidance is offered. Basic surveyor PPE should include: 0 Body protection (hard wearing overalls with suitable pockets for notebook etc). 0 Foot protection (steel toecaps (200 joules), steel midsoles, good grip, oil resistant). 0 Head protection (hard hat with chinstraps) 0 Hand protection (hard wearing gloves). 0 Eye protection (protective glasses, goggles). Ear protection (ear defenders or ear plugs - w
