2、uditing and verification/validation planning by Projects. Configuration Audit and Verification Requirements (including Functional Configuration Audit & Physical Configuration Audit) were not properly specified within the Constellation Program. This resulted in projects, such as the Ground Operations

3、 Project, deciding that the requirements for a Functional Configuration Audit (FCA) and a Physical Configuration Audit (PCA) could be satisfied through a Design Certification Review (DCR). This decision was not fully coordinated across the Constellation Program. Description of Driving Event: Given t

4、he lack of guidance from the Constellation Program in the form of an Audit and Verification Plan, there was a possibility that ambiguous direction from the Ground Operations Project would hinder the verification and successful configuration of audits such as a physical configuration audit or a funct

5、ional configuration audit (to be replaced by DCR and Systems Acceptance Review). Lesson(s) Learned: Programs should provide an audit plan that clearly defines the audits and verifications that are expected. Projects should identify to the Program early on when a requirement will not be satisfied by

6、Project Configuration and Data Management (CDM). Recommendation(s): Programs should produce an audit plan that formally addresses and documents required audits so that any deviation from these requirements are properly documented by Projects in either a variance (waiver or deviation) or applicable d

7、ocumentation (Acceptance Data Package, Master Verification Plan, Systems Engineering Management Plan, etc.).
