Chief, HQ AFMC/SES and

2、 SSEA Team Chief came out on April 19th, 2005 to accomplish the X-45A J-UCAS Hot Refueling System Safety Engineering Analysis (SSEA) for the USAF. During his initial in brief he made a very refreshing statement, “ that he wasnt here to approve, disapprove or inspect, he was here to help us accomplis

3、h this process safe and right.” After the successful completion of the System Safety Engineering Analysis for the X-45A hot refueling he stated they (USAF) would revise T.O. 00-25-172 to list approval of X-45A hot refueling. Until the T.O. is revised, he advised the team that e-mail from his office

4、would provide the authority to conduct x-45A hot refueling operations effective immediately. Description of Driving Event: The X-45A Unmanned Combat Aerial Vehicle (UCAV) had a requirement to hot refuel, (refueling with engine running) the vehicle in order to complete its Block 4 flights. Per Air Fo

5、rce T.O. 00-25-172 (Ground Servicing Of Aircraft and Static Grounding/Bonding) this type of refueling requires the approval of the MAJCOM (Major Command) of the activity involved. Lesson(s) Learned: Even though this System Safety Engineering Analysis was required by the Air Force T.O., bringing in a

6、n expert to have an unbiased look at a new process or procedure especially a person with such a great attitude to get the job accomplished safely and correctly has its merits. 7、iewed the written procedures for the X-45A hot refueling. Mr. Dorney noticed that a standard USAF requirement for the verification of the refueling nozzle being positively locked onto the aircraft receptacle was over-looked. The verification of positive lock is accomplished by attaching the refuel n

8、ozzle to the aircraft receptacle, flipping up the locking handle, and then trying to remove the nozzle, if the nozzle handle rotates open past the locking handle you have a bad refuel nozzle and it shouldnt be used. Recommendation(s): This verification procedure should be briefed to all our aircraft

9、 maintenance personnel involved with refueling our aircraft.
