2、Interfaces Description of Driving Event: A 65-ton mobile crane was hoisting a 40,000 pound test weight from payload canister #2 to a flatbed truck. The test weight was 8 to 10 feet above the canister doors when the boom of the crane slipped, falling toward the canister and lifting the front wheels o

3、ff the ground. The test weight contacted the inside of the canister door and slid inside the canister, damaging the side of the canister. One employee sprained his right foot as a result of the mishap when he quickly jumped from the canister door to avoid being hit by the crane boom.Lesson(s) Learne

4、d: 1. Improper assembly and adjustment of boom hoist brake releasing mechanism may allow the boom and load to lower rapidly and uncontrollably.2. Using improper load charts when determining the load radius will create a hazardous situation that may cause the crane to tip over.3. Neglecting to add th

5、e weight of heavy rigging and the weight of the crane load block will result in a miscalculated load.4.
