2、f the Space Shuttle Program, post-flight reconstruction and performance assessment were reduced as a cost saving measure. This reduction in the scope and detail of post-flight reconstruction and performance assessment precluded the detection of inaccuracies in the definition of induced environments

3、for the design of the Space Shuttle vehicle. Description of Driving Event: During the post-flight reconstruction of STS-133, it was observed that the calculated aerodynamic heating exceeded the design environments for several body points on the external tank and the orbiter during second stage fligh

4、t. It was determined that there had been a software error in the rarefied gas heating model used during certification that resulted in the aerodynamic heating being understated for off-nominal vehicle attitudes. The aerodynamic heating environments were updated for these vehicle elements for subsequ

5、ent flights. Lesson(s) Learned: Detailed post-flight reconstruction and performance assessment is necessary to ensure the adequacy of the space vehicle design environments and the sustained safe performance of the vehicle. Recommendation(s): Do not prematurely reduce the scope and depth of post flig
