2、 repair of a forklift, a 100% load test was being performed with 4,995 pounds of weight. The weight configuration consisted of a large metal weight of 4,745 pounds and several smaller weights on a metal pallet. The load test only requires lifting the weight approximately 12 inches and tilting it up

3、and then down. When the weights were tilted down the 4,745 pound weight slid off the pallet onto the floor and tipped over. There was no injury to personnel and minor damage to the floor. Description of Driving Event: Procedural deficiency: The metal to metal surface of the weight and the metal pall

4、et without strapping the weight in place. Review of data verified that the tilt testing is a contractual requirement outlined in NASA-STD-8719.9 and has to be performed.Lesson(s) Learned: Added contract requirement to Preventive Maintenance Instruction (PMI) instructions. Have a formal review with t
