2、eceived at the Center were not reviewed for possible GIDEP alerts and NASA Advisories. Description of Driving Event: An aircraft (DC - 8) containing Life Support equipment (Life Raft) was received at the Center and a review of possible applicable GIDEP reports was not performed. The Life Raft, after

3、 being overhauled, was located in a storage area and had an inadvertent inflation. It was later determined that the Life Raft contained a valve, for which an applicable GIDEP report had been issued that recommended that the valve be replaced. Although the exact root cause of the inadvertent inflatio

4、n could not be identified, the fact that the valve should have been replaced was identified as a contributing factor. Lesson(s) Learned: A review of GIDEP reports for critical components of new systems received at the Center should be performed. (This may also include procurements where use of GIDEP
