It also permits the procedures to be inspected/reviewed by the type of procedure and considered as a single procedure for the annual inspection/

    14、review in accordance with 6.8.7 and OSHA Standard Interpretation 03-09-2004, “LO/TO periodic inspection requirements.” 6.3.2.6 Equipment/systems powered by the same type electrical panel or same voltage level panel, but do not have similar type controls, or operate differently, or are constructed di

    15、fferently shall not be considered as similar equipment/systems. 6.3.2.7 Energy-control procedures shall only be written by employees that have received training in energy-control safe-work practices in accordance with section 10.2. NOTE: This employee is not required to be listed in CERTRAK or in th

    16、e FMO database for construction as an MSFC or MSFCs MAF authorized employee unless they actually place the LO/TO device and perform the service/maintenance activity. 6.3.3 Organizational standard energy-control safe-work practices. 6.3.3.1 Organizational standard energy-control safe-work practices s

Be documented and inspected/reviewed periodically and not to exceed an annual review in accordance with 6.8 by the organization with all authorized employees that perform LO/TO using the safe-work practices.

    19、b. Clearly state the procedural steps to safely shutdown the equipment/system. c. Clearly state the procedural steps to place the equipment/system in a zero-energy state and release any stored or hazardous energy. d. Clearly state the procedural steps to place and remove LO/TO devices. e. Clearly st

    20、ate the requirements for testing the equipment/system to verify the effectiveness of the LO/TO devices. 6.4 LO/TO Devices 6.4.1 LO devices defined in 5.18 and TO devices defined in 5.28 are used to control energy and shall be substantial enough to prevent their removal without the use of excessive f

    21、orce or unusual techniques, such as with the use of bolt cutters or other metal cutting tools. 6.4.1.1 These devices include, but not limited to, locks, chains, wedges, key blocks, adapter pins, and/or self-locking fasteners. 6.4.1.2 A tag attached to the energy-isolation device can serve as a LO/TO

    22、 only if the energy-isolation device cannot accept any of the devices listed in 6.4.1.1 and approval is obtained in accordance with 6.4.9. 6.4.2 Each organization performing service/maintenance activities shall provide the required LO/TO devices to their authorized employees to safely isolate the eq

    23、uipment/system from its energy sources, and prevent the unauthorized energization of the equipment/system during service/maintenance activities. 6.4.3 All LO/TO devices shall be installed where they are clearly visible, so every employee understands that removing the LO/TO device or attempting to op

    24、erate the equipment/system by moving the energy-isolation device from the “safe” or “off” position to the “on” position is strictly prohibited. 6.4.3.1 Any employee discovered attempting to remove a LO/TO device is subject to disciplinary action described in section 6.9. 6.4.4 Standardized LO/TO dev

    25、ices (locks and tags) at MSFC and MSFCs MAF used for controlling energy on equipment/systems that meet the requirements listed in section 6.1, are described in Chapter 1 of this MWI.     27、led to control energy while an authorized employee is actually working in the danger zone in accordance with section 6.1 of this MWI. 6.4.4.2 Standardized LO/TO devices are not allowed to be used for any other purpose, such as configuration/position locks and tags for purposes defined in section 6.4

    28、.6. 6.4.4.3 Standardized LO/TO devices are to be removed from the energy-isolation source when work is not actually being performed and replaced with a configuration/position lock and tag in accordance with section 6.4.6 if the equipment/system is not in a safe condition to return to operation. 6.4.

    29、4.4 When work resumes on the equipment/system, the authorized employee shall remove the configuration/position lock and tag and replace it with the standardized LO/TO device before starting any work. 6.4.4.5 An exception is allowed to 6.4.4.3 in the following situation: standardized LO/TO devices ar

    30、e allowed to be installed on equipment/systems for a specified period of time during a planned/scheduled power outage, Facility Work Request, Test Preparation Sheet, or other approved work authorization document. The LO/TO device shall be removed at the end of the time period specified by the organi

    31、zation that had the primary responsibility for performing the work. 6.4.4.6 All standardized lockout devices shall be tagged with the standardized tagout device. 6.4.5 Standardized LO/TO devices shall be labeled or contain the information required in Chapter 1. 6.4.5.1 LO/TO device labels and tags s

    32、hall be maintained in a legible condition. 6.4.5.2 Labels and tags that are discovered not to be legible or the information is unreadable shall be replaced. 6.4.6 Configuration/position locks and tags. 6.4.6.1 Configuration/position locks and tags are to be installed when the following conditions ex

    33、ist: a. Work has not been completed on the equipment/system and it is not in a condition permitting a safe return to service by the end of the shift or workday, and the equipment/system is not on a specified outage per 6.4.4.5. Awaiting delivery of

    35、replacement parts that go past the end of the shift or workday. c. A design change is required that goes past the end of the shift or workday. d. To keep the equipment/system in the “off” position if it is not safe to operate. e. To keep the equipment/system in the “on” position to prevent someone f

    36、rom inadvertently turning it off. f. To keep unauthorized employees from operating the equipment/system. g. Locking of the energy-isolation device is required to support safe removal of “abandoned in place” items from periodic maintenance and inspection schedules. 6.4.6.2 MSFC and MSFCs MAF do not h

    37、ave a standardized configuration/position lock. The lock used to maintain configuration/position of the equipment/system can be any size or shape required to safely lock the equipment/system in the desired position. The configuration/position lock shall not have a “red” case. 6.4.6.3 MSFC does not h

    38、ave a standardized configuration/position tag. The tag shall comply with MWI 8715.3, “Hazard Identification and Warning System” and be appropriate for the hazard level for which they are being placed. The hazard level is determined by the employee placing the tag. 6.4.6.4 All locks used to maintain

    39、configuration/position of equipment/system shall be tagged with a tag appropriate for the hazard level as identified in MWI 8715.3, “Hazard Identification and Warning System.” 6.4.6.5 Configuration/position tags are to include the following information: a. The organization name or MSFC or MSFCs MAF

    40、organization code. b. Name of the employee that placed the lock or tag. c. Contact phone number. d. A brief description of why the tag was installed. 6.4.6.6 A lock is not always required to be installed with a configuration/position tag. The installation of a lock shall be based on the hazard level

    41、 as determined by the employee installing the tag. It is highly recommended to always install a lock if the energy-isolation device is capable of accepting a locking device.     43、. 6.4.6.8 Tags that are discovered not to be legible or the information is unreadable shall be replaced. 6.4.6.9 Standardized LO/TO devices shall be removed and replaced with configuration/position locks and tags for any of the reasons listed in 6.4.6.1. 6.4.7 Samples of standardized LO/TO devices s

    44、hall be displayed by the ISB, FMO, and/or other onsite organizations responsible for requesting offsite contractors to perform work on MSFC during contractor in-briefings, when requested. 6.4.8 Standardized lockout and tagout devices can be obtained from: 6.4.8.1 The MSFC Retail Store. This store is

    45、 for all MSFC organizations and onsite contractors approved for purchase at MSFC. More information on obtaining supplies from this store can be obtained from the Center Operations Web page or by contacting the Logistics Services Office. 6.4.8.2 Local vendors. 6.4.8.3 At MSFCs MAF the Central Commodi

    46、ties Crib. 6.4.9 Energy-isolation device not capable of being locked. 6.4.9.1 Tagout devices, when used without a lock for the purpose of LO/TO, shall not be allowed unless the following can be met: a. It is demonstrated that the energy-isolation device is not capable of being locked, and a tagout d

    47、evice alone can provide full employee protection in accordance with Chapter 5. b. Approval is obtained from the authorized employees supervisor, or ISB, or the organization that requested the service/maintenance to be performed. FMO approval is required for construction activities. 6.4.9.2 The autho

    48、rized employee shall take additional safety measures such as removing fuses or lifting wires to provide full employee protection. 6.4.9.3 The additional safety measures are to be noted on the tag. 6.5 Group LO/TO Provided by IHSNot for ResaleNo reproduction or networking permitted without license fr
