    3、for the qualification and certification of person-nel who perform inspection of coating and lining work.Establishment of qualification requirements to verify confor-mance to specified requirements for nuclear facility coatingand lining work is necessary to assure satisfactory performanceof the inspe

    4、ctions and to avoid compromising safety-relatedcoating systems.1.2 The intent of this guide is to provide a uniform inter-pretation of the requirements in ANSI/ASME N45.2.6 orANSI/ASME NQA-1 as applicable, for the inspection ofcoating and lining work in nuclear facilities.1.3 It is the intent of thi

    5、s guide to provide a recommendedbasis for qualification and certification, not to mandate asingular basis for all qualifications. Variations or simplifica-tions of the qualifications described in this guide may beappropriate for special coating and lining work other thansafety-related coating and li

Similarly, the quali-fication and certification process might be abbreviated for workof minor scope such as touch-up.1.4 This standard does not purport to address all of thesafety concerns, if any, associated with its use. It is theresponsibility of whoever uses this standard to consul

Terminology3.1 DefinitionsDefinitions for use with this standard areshown in Terminology D4538 or other applicable standards.3.2 Definitions of Terms Specific to This Standard:3.2.1 certification, nwritten documentation of qualifi

    10、ca-tion.3.2.2 coating and lining work inspection, na phase ofquality control that, by means of examination, observation, ormeasurement, determines the conformance of safety-relatedcoating or lining system applications to predetermined qualityrequirements. The term inspection as used in this standard

    11、 shallbe understood to mean coating and lining work inspection.3.2.3 Level I, II, and III inspection, nindicative of thetime in grade classification for personnel who perform coatingand lining work inspection. Each level reflects progressiveexperience and responsibility. This term is separate from a

    12、nddiffers distinctly from Coating Service Levels. The latter termreflects differing locations throughout a nuclear plant wheresafety-related and other coatings are found. (See Terminologysection of Guide D5144 for definitions of Coating ServiceLevels I, II, and III.)3.2.4 personnel performing coatin

    13、g work inspection,nindividuals whose job functions include but are not neces-sarily limited to coating and lining work inspection.3.2.5 qualifications, nskills, training, and experience re-quired for personnel to perform properly the duties and executethe responsibilities of the appropriate certific

    18、e knowledge and skills necessary forqualification.4. Significance and Use4.1 The requirements of this guide apply to personnel whoperform coating and lining work inspection during (1) fabri-cation, (2) receipt of items at the construction site, (3)construction, (4) pre-operational and startup testin

    19、g, and (5)operational phases of nuclear facilities.4.2 It is the responsibility of each organization participatingin the project to ensure that only those personnel within theirrespective organizations who meet the requirements of thisguide are permitted to perform coating and lining workinspection

    20、activities covered by this guide.4.3 The organization(s) responsible for establishing theapplicable requirements for activities covered by this guideshall be identified, and the scope of their responsibility shall bedocumented. Delegation of this responsibility to other qualifiedorganizations is per

    21、mitted and shall be documented.4.4 It is the responsibility of the organization performingthese activities to specify the detailed methods and proceduresfor meeting the requirements of this guide, unless thosemethods and procedures are otherwise specified in the contractdocuments.4.5 In the event of

    22、 conflict, users of this guide mustrecognize that the licensees plant-specific quality assuranceprogram and licensing commitments shall prevail with respectto the process of qualifying personnel performing inspection ofcoating and lining work.5. General Requirements5.1 Provisions shall be made for t

    23、he indoctrination ofpersonnel performing coating and lining work inspection as tothe technical objectives of the project, the codes and standardsthat are to be used, and the quality assurance elements that areto be employed.5.2 The need for and content of formal training programsshall be determined,

    24、 and such training activities shall beconducted as required to qualify personnel who performcoating and lining work inspection. The extent of on-the-jobparticipation (“time in grade”) also shall be addressed in theprogram, with emphasis on first-hand experience gainedthrough actual performance of in

    25、spections.5.3 A candidates qualifications for certification shall beinitially determined by a suitable evaluation of the candidateseducation, experience, training, examination results and by acapability demonstration. The qualifying organizations deter-minations must reflect definitive criteria with

    26、 respect to theextent and currency of a candidates experience and training.5.4 The job performance of personnel who perform coatingand lining work inspection shall be reevaluated at periodicintervals not to exceed three years. The reevaluation processshall weigh evidence of continued satisfactory pe

    27、rformance ofinspection for the required extent or determine capability inaccordance with 5.3. If, during this evaluation or at any othertime, it is determined by the responsible organization that thecapabilities of an individual are not in accordance with thequalifications specified for the job, tha

    28、t person shall be re-moved from that activity until the required capability has beendemonstrated.5.5 Qualification is considered revoked for any person whohas not been actively engaged in the performance or supervi-sion of coating and lining work inspection for a period of oneyear during the certifi

    29、cation period. Reevaluation in accor-dance with 5.3 is required to restore qualification.6. Functional Qualifications6.1 All physical coating work inspection activities can beperformed by personnel certified as qualified to performcoating and lining work inspection as Level I, Level II, orLevel III

    30、status as described in the following.6.2 Personnel certified as having attained Level I status shallbe capable of the following:6.2.1 Implementing and recording all inspections requiredby the applicable procedures.6.2.2 Verifying instrument calibration.6.2.3 Performing hold point inspections in acco

    31、rdance withthe applicable procedures.6.3 Personnel certified as having attained Level II statusshall be capable of the following:6.3.1 Performing all of the duties and responsibilities ofLevel I status personnel.6.3.2 Planning and supervising inspections, initiating andreviewing inspection procedure

    32、s, and evaluating the adequacyof inspection activities performed.6.3.3 Reviewing, organizing, and approving results of in-spections.6.3.4 Monitoring the performance of and supervising thework of Level I status personnel.6.3.5 Training and verifying, for certification, the qualifica-tions of candidat

    33、es for Level I inspection.6.3.6 Initiating changes to quality procedures.6.3.7 Implementing the Quality Assurance Program if as-signed that authority by company policy or the QualityAssurance Program.6.3.8 Reviewing and, to the extent permitted under project/plant procedures, documenting approval of

    34、 quality relatedaspects of safety-related coating and lining specifications.6.4 Personnel certified as having attained Level III status,shall have verifiable experience in the U.S. commercial nuclearpower industry. As indicated in 5.3, criteria with respect to theextent and currency of that experien

    35、ce must be established.Further, the following capabilities are required:6.4.1 Carrying out all of the duties and responsibilities ofLevel II status personnel.6.4.2 Training and verifying, for certification, the qualifica-tions of candidates for Level I, Level II, and Level IIIinspection.6.4.3 Evalua

    36、ting the adequacy of programs used to train andcertify personnel who will perform coating and lining workinspection.D4537 1226.4.4 Authorizing personnel certified to perform Level IIinspection to carry out training and examination duties.6.4.5 Approving safety-related coating and lining workinspecti

    37、on procedures.7. Physical Qualifications7.1 Individuals who perform coating and lining work in-spection shall be examined annually to ensure natural orcorrected near-distance visual acuity in at least one eye. Theindividual shall read the J-1 letters on a Standard Jaeger TestChart, or equivalent, at

    38、 a distance of not less than 12 in. withone or both eyes, uncorrected or corrected.7.2 Each candidate shall be examined for color perceptionusing the Ishihara Test or the Farnsworth D-15 Test when beingcertified or recertified. If a candidate does not pass theRed/Green Sensitive Ishihara Test, the c

    39、andidate may take theFarnsworth D-15 Test.7.3 If a candidate does not pass the Farnsworth D-15 Test,the candidate may be evaluated by a licensed medical practi-tioner to provide the necessary data to determine the candi-dates color perception. An individual certified after an evalu-ation by a licens

    40、ed medical practitioner may only be certifiedto perform inspection work that is within the candidatess colorperception capability.7.4 The examinations required by 7.1 and 7.2 shall beadministered by a licensed medical practitioner or a personfamiliar with the tests involved. The results of vision te

    41、sts shallbe documented on a Vision Test Record (Fig. 1 or equivalentform).7.5 The responsible organization shall identify any otherphysical qualifications required to perform the assigned inspec-tion duties. Personnel performing inspection activities requir-ing the identified physical qualifications

    42、 shall have thosequalifications confirmed by examinations at intervals not toexceed one year.8. Education, Training, and Experience Qualifications8.1 Candidates for certification to perform coating andlining work inspection shall have sufficient education, experi-ence, and training to ensure an unde

    43、rstanding of the principlesFIG. 1 Sample Vision Test RecordD4537 123and procedures in those areas of inspection, examination, andtesting activities for which they are being considered forcertification.8.2 Candidates for certification to perform Level I inspec-tion shall, as a minimum, meet one or mo

    44、re of the followingrequirements:8.2.1 High school graduation or equivalent plus six monthsof related experience in equivalent inspection activities.8.2.2 Completion of college level work leading to anAssociate Degree or higher, plus three months of relatedexperience in equivalent inspection activiti

    45、es.8.3 Candidates for certification to perform Level II inspec-tion shall, as a minimum, meet one or more of the followingrequirements:8.3.1 High school graduation or equivalent plus one year ofsatisfactory performance of Level I inspection in a correspond-ing inspection activity.8.3.2 High school g

    46、raduation or equivalent plus three yearsof related experience in equivalent inspection activities.8.3.3 Completion of college level work leading to anAssociate Degree plus one year of related experience inequivalent inspection activities.8.3.4 Four-year college graduation plus six months of re-lated

    47、 experience in equivalent inspection activities.8.4 Candidates for certification to perform Level III inspec-tion shall meet one or more of the following requirements:8.4.1 High school graduation or equivalent plus six years ofsatisfactory performance of Level II inspection in a corre-sponding inspe

    48、ction activity.8.4.2 High school graduation or equivalent plus ten years ofrelated experience in equivalent inspection activities; or highschool graduation plus eight years experience in equivalentinspection activities, with at least two years satisfactory per-formance of Level II inspection. The ca

    49、ndidate shall have atleast two years associated with nuclear facilities or sufficienttraining to be knowledgeable of the quality assurance require-ments for nuclear coating work.8.4.3 Completion of college level work leading to anAssociate Degree and seven years of related experience inequivalent inspection activities. The candidate shall have atleast two years of this experience associated with nuclearfacilities.8.4.4 Four-year college graduation plus five years of relatedexperience in equivalent inspection activities. The candidateshall have at least two years of
