2、ccurred as it was under going pre-flight for a mission the following day. During the preflight, a gaseous nitrogen servicing hose was pressurized to approximately 5200 psi without the servicing end connected to the aircraft. The hose end rose up and struck the aircraft numerous times, puncturing the
3、 upper surface of the right flap in two places. The flow valve was shut off when the hose became lodged under a servicing cart tire. No one was injured. Description of Driving Event: The servicing hose was not secured (safety lanyards or sandbagged) nor connected to the aircraft when it was charged
4、with gaseous nitrogen. Lesson(s) Learned: 1. All high-pressure hoses must be connected before pressurizing.2. All high-pressure hoses must be restrained with either safety lanyards or sandbags prior to pressurizing.3. When stored, all high-pressure hoses should have high-pressure caps installed.Reco
